• 1 hour = $150
• 90 minutes = $225

Cash, check and credit cards accepted at time of service.

Forms

Our first visit requires that we sign several forms prior to getting started. Please download the file to the right and sign all 6 forms, and bring them with you to our first session. The financial form also requires initials in two places. The Service Tracker form requires you to fill in your contact and other information. If you do not have access to a printer I can provide the forms for you. This will take five or ten minutes out of our first session.

No Surprises Act

No Surprises Act: Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items or services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
